Grant recipients have a responsibility to complete LEEF review forms. Please download, fill out and submit the mid-year and year-end reports contained in the microsoft file below.

Assessment Form Instructions

Submit a Mid-Year Progress Report by January 30th and End-of-Year Assessment by May 1st to the Grant Shepherding Committee.
